Service Coordination

Service coordination is the responsibility of the service coordinator. Responsibilities include supporting the individual in the person centered planning process including preplanning which includes community mapping, satisfaction survey, who to invite, review of goals to name a few. The service coordinator is also available to assist with housing issues, roommate selection, medical services, transportation and other services that support in the individual in accomplishing goals, needs and dreams.
